Acknowledgement of Risk and Voluntary Participation
I understand that my participation in any Firewalking, Glasswalking, Legowalking, Arrow Breaking, Board Breaking, or any other empowerment activity provided by Firewalk Events is entirely voluntary and undertaken at my own risk.
I acknowledge that while Firewalk Events follows strict safety procedures, no guarantee of absolute safety can be given.
I agree to hold harmless and indemnify Firewalk Events, its organisers, facilitators, sponsors, staff, volunteers, and property owners from any and all liability, claims, or damages arising from my participation.
Health & Fitness Declaration
I confirm that, to the best of my knowledge:
I understand that participation is entirely optional, and I may withdraw at any point before or during the activity without consequence.
Acknowledgement of Inherent Risks
I understand that the activities provided by Firewalk Events involve inherent risks, including but not limited to:
I acknowledge that serious injury can occur, and that medical attention may be required.
By signing below, I voluntarily assume all such risks and waive my right to bring any claim against Firewalk Events or its representatives for physical, emotional, or financial damages.
Mental Wellbeing
I confirm that I am mentally sound and of stable mind to make this voluntary decision.
I understand that if I have a mental health condition that could be affected by stress or empowerment-based activities, it is my responsibility to seek independent medical or therapeutic advice before participating.
Photography, Media & Data Protection (UK GDPR 2018)
I understand that photos and/or video footage may be taken during this event for promotional use by Firewalk Events.
If I do not wish my image to be used, I will inform a staff member before the event begins.
Firewalk Events complies with the UK GDPR and will store my personal information securely.
My data will only be used for event administration, safety purposes, and (if consented) occasional marketing updates.
My information will not be shared with third parties without consent.
Emergency Declaration
I agree to follow all instructions from Firewalk Events staff and understand that safety guidelines are in place to protect all participants.
In the event of an emergency, I authorise trained staff to provide first aid or request medical assistance as deemed necessary.
Participants Under 18 Years of Age
If the participant is under 18, this form must be completed and signed by a parent or legal guardian who accepts full responsibility for the participant’s wellbeing and releases Firewalk Events and all associated parties from liability.
Firewalk Events Declaration
Firewalk Events operates in accordance with UK health and safety legislation, including the Health and Safety at Work Act 1974, Management of Health and Safety at Work Regulations 1999, and UK GDPR 2018.
All instructors and facilitators are trained professionals and conduct events under strict safety procedures and risk assessments.
